Description

At Twizell Colliery Durham, an onsetter was killed by falling part way down the shaft. Deceased, about to travel from one mid-inset to another higher in the shaft, stood with one foot in the cage and another on the inset whilst he gave the necessary signals. After giving the cautionary signal of three raps he failed to allow the signalling lever to go back far enough to cancel this signal before rapping "7" to indicate the level to which the cage was to be sent. The signals accumulated on the engineman's indicator to "10", which was the signal fixed for coal work. When this signal was followed by the action signal of "1" the engineman took this for "cage clear" and, in accordance with the usual custom, immediately lowered the cage so as to bring the other cage to bank for empty tubs. Deceased was precipitated down the shaft and instantly killed. He should have stood in the inset whilst giving the signals, and should not have set foot in the cage before receiving from the banksman a reply to his cautionary signal.
