A driller at Usworth Colliery, Durham, was removing a trailing cable from a longwall conveyor face, when his foot inadvertently pushed the cover plate partially off the top of the sheet iron box protecting the return end of a Blackett gateway conveyor. Shortly afterwards when leaving the face he accidentally stepped into the box. His leg was drawn into the conveyor chain bearing down pulley and crushed against the pans. He sustained fatal injuries. Three of the four top bolts securing the plate were missing. These had probably been forced out by a piece of wood becoming entangled in the conveyor chain. The machinery had been examined on the previous day when all the top bolts were in position and it was known that the plate was not displaced a few minutes before the accident.
To prevent a repetition of a similar accident, a cover plate with turned down flanges, secured with bolts and check nuts, was fitted.
